We are glad you’re going to begin your foot health journey with us. Your first visit here is important. It will help establish a foundation for your relationship with our office. We understand that people visit us because they have real concerns about their foot and ankle health. New Patient Forms

Your First Visit

When you arrive for your first appointment, we will greet you and sign you in. At this time the reception staff will need to make a record of your contact and insurance information. We understand that filling out these forms takes time, so we have provided them here for you to download and fill out before you come in, if you would prefer to do so. Please remember to bring a photo ID and your insurance cards as well when you visit.

During your appointment, we’ll record your background and medical history. That will give us a chance to not only get to know you better, but also make us aware of any past conditions and treatments you may have had that could play a role in your current health. You will also have a chance to get to know us a little better, as well as express your questions and concerns about your feet and ankles. Through this, we aim to eliminate any anxiety you may have and obtain the information we need to best treat you.

Insurance and Payment Options

We understand that insurance is complicated and plans vary by providers. It can be hard to know what a doctor will or will not accept, or what your insurance company will or will not cover. Our office participates with Blue Cross, Medicare, and most commercial providers. We will do our best to assist you in investigating what your plan does and does not cover when you visit our practice. If your insurance requires you to have a referral to visit a specialist, like HMO patients, please obtain one from your family practitioner before you come in to see us, and bring it with you to your appointment if applicable. If you do not, you may have to reschedule—or cover the cost of the visit yourself if insurance denies payment.

You are also responsible for any and all copays or deductibles at the time of service, so please remember to bring some form of payment with you. We accept Care Credit, Mastercard, Visa, Discover, and American Express, as well as cash and checks.
