Original x-rays are not released to patients and a charge of $5 per sheet is required to copy x-rays.
If a patient has no insurance or their insurance requires a co-pay, payment is expected at the time of service.
No patient under 18 will be treated without permission of a parent or guardian.
Patients with an insurance requiring referrals are responsible for contacting their Primary Care Physician before their appointment and must have them at the time of their appointment or they will have to reschedule.
Patients are required to sign surgical consent forms before a surgery is performed and after the physician explains the procedure and risks involved in detail.
When a patient has a complaint about a service or bill, the appropriate person is contacted and if they are not satisfied with their explanation then further help is obtained.

We want you to know that all of our employees, managers and doctors continually undergo training so that they may understand and comply with government rules and regulations regarding the Health Insurance Portability and Accountability Act (HIPAA) with particular emphasis on the “Privacy Rule.” As of April 14, 2003, all patients are required to review and sign a notice of privacy practices.
